New Patient Form
Please complete the new patient form and bring it with you to your initial appointment, or arrive 10 minutes prior to your appointment time to complete one at the practice.

ACROD Parking Permit Application
If you are entitled to apply for an ACROD parking permit, please complete the patient’s section of the application form and bring it with you to your consultation.
Taxi Voucher Application
If you are entitled to apply for Taxi Vouchers, please complete the patient’s section of the application and bring it to your consultation.
Navigate to: Taxi/Charter → Passenger Information and Complaints → Passenger Subsidies.
Centrelink / Medicare
Not well enough to attend in person?
Our GPs offer phone consultations if you are unwell, or have COVID or COVID and flu symptoms. Phone consults are charged at a private fee and are not bulk billed regardless of age. You must have attended the practice face to face within the last 12 months.
